Hi Dr. Horvitz! I think you missed my take on ketoacidosis. I say, "the levels of ketones in diabetic ketoacidosis are 5 to 10 times higher than the levels in dietary ketosis." I've never heard of ketoacidosis occurring with a keto diet, but dietary ketosis is common and often the point of the diet for epileptics.
Dietary ketosis MAY be problematic to a diabetic on the edge, so I recommend they talk to a doc, just in case.

Ketoacidosis occurs when insulin is very low or non existent, such as in Type 1 Diabetics.
The treatment for Ketoacidosis is provide insulin, rehydrate and rebalance the other minerals and electrolytes.
Type 1 diabetics who use insulin will not get Ketoacidosis as long as they use insulin.
Type 2 diabetics have very high insulin levels so a keto diet will help bring those levels back to healthier levels, but not too low where they would develop Ketoacidosis.
Your info on gut and microbes is very interesting and I will be taking a deeper dive into this as I agree it is going to be the wave of the future.
